Symptom Screening App

Athena has available a QR Code to point to your account's customizable symptom screening app.

For Example:

2) This link to the web questionnaire will open up the customized URL that hosts your organization' symptom screening questions.

3) User may be prompted to enter First and Last name and complete the questionnaire by selecting Yes or No.

4) User receives a pass or fail and the pass that can be displayed as an Attestation, and fails can generate an alert notification.

5) Admin users have access to reporting and exporting as .csv files.
